Preferred vs. Non-Preferred Providers
You can use any dentist with Landmark Personal Plan coverage. You benefits are greater when you use a dentist (Preferred Provider) within our network. Search our Preferred Provider Dental Network.
How old do I have to be before I can enroll?
A policy holder must be 18 years of age or older
What kind of payment options do I have?
Automatic bank withdrawal or Visa\MasterCard

Who can be considered my dependent?
A spouse. Also, each unmarried child from birth to age 19 who lives with you in a regular parent-child relationship that you can claim on your taxes. Each unmarried child from age 19-23 who is a full time student who is enrolled for at least 12 semesters hours. Each unmarried child at least 19 years of age who is incapable of self sustaining employment by reason of mental retardation or physical handicap, who was incapacitated and insured under the policy on his 19th birthday and who continues to be incapacitated beyond his 19th birthday.

Enrollment materials will be mailed to you within 10 days of a completed registration form.
How often do rates change
Rates may change with 30 days notice.
